Johnson VA medical center in Charleston, SC. use reverse andor attach additional sheets as necessary. repair and service: all repairs and service to heating, air conditioning, chiller, lift gate, roll door, lighting interior and exterior of coach, humidifier, air ride suspension, belly compartments and doors, ceiling tile, roof, roof leaks and generator. srs will have a phone response time of 4 hours and an onsite response time of 24hrs. preventive maintenance services: a preventative maintenance will be performed four times a year at three- month intervals. the pm will be performed on the entire unit, excluding the generator. summary of pm attached a pm to the generator will be performed at 300 hours running time or at least once per year and is included in the contract cost. contra

Offerors must provide a quote for the base and option years. Additional documents (preventive maintenance procedures manual) are required within 15 calendar days after award date. Payment will be made upon completion of service. The contract includes clauses for contract terms and conditions, option to extend services, supplemental insurance requirements, electronic submission of payment requests, administrative contracting officer, clauses incorporated by reference, and contract terms and conditions required to implement statutes or executive orders. The contract also specifies locations, terms of agreement, access to equipment, warranty, contractor personnel security requirements, interference to normal function, contracting requirements, hours of work, additional services, authorized services, discontinuance of service, test equipment, performance conference, special contract requirements, and contractor submittals. Obsolete machines are not covered unless mutually agreed upon. The contractor's repairman will report to the contracting officer's technical representative (COtr) prior to start of work and after completing services. The government will make the equipment available during normal business hours (8:00 AM - 5:00 PM, Mon-Fri). If work needs to be done after hours, the government will make arrangements. The contractor must comply with all VA, Joint Commission, OSHA, and NFPA regulations. The contract incorporates various FAR clauses, including those related to contractor employee whistleblower rights, personal identity verification, system for award management maintenance, commercial and government entity code maintenance, insurance, commercial advertising, personal identity verification of contractor personnel, evaluation factor commitments, and others. The contract also includes clauses related to privacy training, buy American supplies, trade agreements, restrictions on certain foreign purchases, and others. The contract is subject to the Service Contract Act, and wage determinations are provided. The contract specifies fringe benefits required, and the applicable minimum wage rate.
