Documentation, including setup and education records, must be submitted within 24 hours of service completion. Home assessments and DME education are required. Contractor must maintain a 24/7 toll-free number for customer service. Reporting requirements include PPC forms, AMA forms, veteran rights and responsibilities forms, damage reports, inventory reports, service satisfaction reports, timeliness reports, and infection control reports.
Deliveries shall be made within 3 business days of receipt of the prosthetics department order. Emergent services may have shorter timeframes.
Payments shall be made in arrears upon receipt of a properly prepared invoice, submitted within 10 days of order placement. Invoices must be submitted electronically to the Veterans Affairs Financial Services Center (VAFSC) via Tungsten Network.
The contractor warrants and implies that the items delivered hereunder are merchantable and fit for use for the particular purpose described in this contract.
Contractor employees must have competency-based training records in all assigned DME tasks prior to performing services. They must also have all required immunizations compliant with state and local health regulations. Background checks are required for all personnel.
Failure to comply with facility requirements shall be considered a material breach of contract and may result in charges answerable in the United States District Court. Violations of VA regulations may result in citation.
Failure to comply with contract terms and conditions, or failure to provide adequate assurances of future performance, may lead to termination for cause.
The estimated total value for the entire duration of the contract is $7,500,****.
