This is a blanket purchase agreement (BPA) for postvasectomy and semen analysis services for the Rocky Mountain Regional VA Medical Center. The agreement is effective from September 1, 2025, to August 31, 2030. The estimated annual quantity for postvasectomy testing is 550, and for full semen analysis testing is 1155. The contractor must be approved to provide clinical diagnostic laboratory tests under both Medicare and Medicaid regulations and all applicable federal and state laws. The contractor shall meet the performance characteristics for accuracy and precision as defined by the **** Clinical Laboratory Improvement Act (CLIA) and the Clinical and Laboratory Standards Institute (CLSI). A valid, in-date CLIA certificate must be supplied. The contractor shall be HIPAA compliant. Invoices must contain specific information, and test results will be provided via fax and the contractor's website. The government may extend the term of this BPA by written notice to the contractor at any time prior to the expiration of the BPA, provided that the government shall give the contractor a preliminary written notice of its intent to extend at least 30 days before the BPA expires. The government estimates, but does not guarantee, that individual BPA orders placed against this agreement may reach $120,**** per order. This order limit may be increased by mutual agreement of the parties as necessary, in whole or part. The authorized ceiling limit of the agreement is set at $660,**** over the period covered by the agreement. The contractor shall comply with all applicable federal, state, and local laws, executive orders, rules, and regulations applicable to its performance under this contract.
