This is a one-time group buy for 23 items of stationery and record forms. The procurement is set aside for 100 small businesses under NAICS codes **** and **** must be compliant with TAA and MIL STD 129R. The notice is for sources sought only and will not be used to make an award.

This solicitation is for **** wallboard, building paper, and thermal insulation materials. Electronic submission of quotes is available through the beta sam website. To be considered for award, the contractor must meet certain requirements, including a current NIST SP **** assessment (if applicable). Late bids will not be considered. The solicitation must be completed in its entirety and returned. A signed and completed solicitation must be submitted via email or fax. The solicitation is for the supply of medical equipment and supplies to Crownpoint Health Care Facility. Offerors must complete blocks 12, 17, 23, 24, and 30. Quotes determined to be incomplete, unreasonable, or unrealistic will not be considered. Sellers must enter exact bid information, including brand and description, into the description field. Minimum bid decrement is 100. Purchase order or delivery order will be issued. Registration with SAM (System for Award Management) is required. No partial shipments are permitted unless authorized. Delivery must be made within 30 days. New equipment only, no remanufactured or gray market items. All items must be covered by manufacturer's warranty. Bid must be good for 30 calendar days after close of buy. Shipping must be free on board (FOB) destination CONUS. Pricing must be inclusive of all line items.
