This information will carry forward to a contract if awarded, therefore please ensure all information is correct. ** Entire Official Bid Document and Bid Bond ( if required) should be combined as one PDF and uploaded** The site is located along Abner Fork Road Spur 1 ( Johnson Coal Hollow) road near the community of Melvin in Floyd County. The citizens residence and yard area are being impacted by eligible mine drainage. This project will install subdrain along the edge of the citizens driveway/yard areas to collect subsurface drainage. Collected drainage will be routed via a drop box into a culvert across the citizens yard outletting into a concrete block- tied mat ditch. The ditch will conduct drainage down the slope to new culvert with headwalls crossing Johnson Coal Hollow road and outlet into Abner Fork stream.

1 provide labor and necessary equipment to remove all filter media anthracite coal, greensand, and gravel then replace the removed and additional media as needed to refill after inspection and maintenance of tanks. pressure wash the interior of the filter tanks when empty for inspection by hungerford and terry. include cost to remove and replace all sand valves in 5 pressure filters with the valves provided by the city of hutchinson. this will likely require concrete to be removed from around the base of each valve pf 4 valves have been replaced previously. 2 provide a separate bid price for the interior recoating of any tanks, after pressure washed and inspected any tank that is found that needs to be recoated. 3 provide separate bid price for pressure filter 4 if any of the sand valves end up needing to be replaced
