This solicitation is for Nicotine Polacrilex Mini Lozenges. Offerors must submit a price for the base year and all four one-year options for all line items. Prices offered shall not exceed two decimal places. Offered prices must include the cost recovery fee of 0. 5%. Offerors must list an 11-digit NDC/UPC number for each offered drug. Proposals will be accepted in Microsoft Word or PDF format via email. Faxed proposals are not acceptable. The government will evaluate offers in accordance with the policies and procedures of the far part 12, part 15, and part 25. The offeror must maintain the same manufacturer nda/and/or bla for the duration of the contract period, unless the contracting officer makes a determination that a change in the manufacturer is acceptable. The contractor shall report all contract sales under this contract and submit collected cost recovery fees. The contractor shall use only the fdainspected manufacturing facilities provided in section 13. 7, below, for the duration of the contract, unless substitution of manufacturing facilities is approved by the contracting officer. All pharmaceutical products provided under this contract shall include bar code labeling at the unitofuse package level. The government reserves the right to terminate this contract, or any part hereof, for its sole convenience. The government may terminate this contract, or any part hereof, for cause in the event of any default by the contractor, or if the contractor fails to comply with any contract terms and conditions, or fails to provide the government, upon request, with adequate assurances of future performance. The Department of Veterans Affairs intends to enter into a sole source procurement with Pharmalogic Ltd for radiopharmaceutical products. Interested parties that believe they can satisfy the requirement must clearly identify their capability in writing by Wednesday, December 31, 2025, at 4 PM EST.
