This is a request for quote (RFQ) for polyethylene foam planks. Quotes must include firm fixed unit prices for all CLINs. The government intends to award one contract. The acquisition is a 100% SBA certified woman-owned small business set-aside. Offerors must be registered in the System for Award Management (SAM) database by the quotation submission date. Pallets must meet specific requirements (****, Revision C). Proof of delivery documents are required (shipping documentation, materials inspection, receiving report DD250, bill of lading, packing list, tracking/delivery confirmation, or invoice). Invoicing and payment will be made via Wide Area Workflow. The contractor must mark the contract or purchase order number on all paperwork and shipments. The order number must appear on the exterior of the shipment. Failure to follow these instructions will hold up payment and could result in the return of merchandise at your expense. The contractor shall be ineligible from participation as a contractor, subcontractor, or consultant in any procurement arising or resulting from any of the services provided to DLA on this contract. The offeror must quote shipment of supplies in accordance with the pallet requirements. The government will award one contract to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The government intends to award one contract to the responsive, responsible offer using the lowest technically acceptable price process, with the lowest overall price for all clins. Award will be made on all or none basis. Technical tradeoffs will not be made, and no additional credit will be given for exceeding acceptability. Award will be based on the overall lowest evaluated price of the quotes meeting or exceeding the acceptability standards for the non-price factors. Any offeror who has submitted a technically acceptable quote and who has been found to have an acceptable past performance will then have the quote evaluated for lowest price. This competition is not being conducted using FAR part 15 procedures. The award decision process is being conducted under FAR part 12 and FAR subpart 13. The government does not intend to conduct discussions but reserves the right to hold discussions if deemed necessary by the contracting officer. The government reserves the right to seek clarifications, i. e. , limited exchanges between the government and interested bidders that may allow bidders to clarify certain aspects of their quotes or to resolve minor or clerical errors. The government reserves the right to evaluate quotes and may seek to make an award without asking for revised quotes. Offerors must complete representations and certifications in the System for Award Management (SAM) prior to submission or provide a completed copy with quote submission.

The solicitation is for a blanket order contract to provide polyethylene foam rolls to the Louisiana Department of Corrections, for an initial period beginning with the date of award and ending December 31, 2025. The contract may be renewed for two additional 12-month periods at the same prices, terms, and conditions, with a total contract time not to exceed 36 months. Items are to be called for by the using agency as needed. Bidders must be registered in the lagov system. A W9 form is required for newly registered vendors. All attachments must be printed and reviewed. Bids must be submitted online via the vendor portal or in a sealed envelope marked with the bid opening date and number. Unit price should include freight charges. The state reserves the right to reject vendors who cannot make delivery within the stated timeframe. The state intends to award the contract to the lowest responsive, responsible bidder meeting the specifications. The state further reserves the right to reject individual line items from the award. Delivery terms are called for as needed by agency basis. Bidders must specify brand, model number, and compliance with federal specifications. Bidders proposing an equivalent brand or model should submit descriptive information. Bid prices must be firm for a minimum of 30 days. Vendors are responsible for including all applicable taxes in the bid price. All products must be new. Contract cancellation may occur for misrepresentation, fraud, conflict of contract provisions, abusive conduct, or intentional violation of the procurement code. The state may terminate the contract for convenience with 30 days' written notice. Failure to deliver within the time specified constitutes a default. All contracts shall be construed in accordance with Louisiana law. Bidders must comply with civil rights laws, ADA accessibility requirements, and other relevant laws. Evidence of authority to sign and submit bids is required. The state is authorized to reject proposals from businesses where individuals with ownership interests of 5% or more have been convicted of certain crimes. The contractor agrees to adhere to the Copeland Anti-kickback Act, Clean Air Act, Clean Water Act, Energy Policy and Conservation Act, anti-lobbying and debarment act, and other relevant federal statutes. The bidder certifies that they have considered all proposals from qualified subcontractors and suppliers and have not discriminated against any person or entity engaging in commercial transactions in Israel or Israel-controlled territories. The bidder certifies that their company, any subcontractors, or principals are not suspended or debarred by the GSA. All bid amounts shall be submitted in US dollars. Independent price determination is required.
