This is a solicitation for pharmaceutical products. The solicitation requires the submission of a signed **** solicitation cover page, a quote price schedule in Excel format, a state wholesale distributor license, and a completed Buy American certificate. Quotes must include confirmed quantities ready for shipment. Delivery terms are specified, including the right to cancel quantities not delivered on time. All shipments must reference the original VA obligation number. Delivery may be required to multiple delivery points. The vendor must email tracking information to the points of contact upon shipment. Bar code labeling requirements are detailed, including symbology, quality grade, and placement. Only products with a therapeutic equivalence code 'A' from the FDA will be considered, unless all drugs in the family group are 'B' rated. If a drug recall is initiated, the vendor must forward copies of the recall notification to specified VA offices and authorized ordering activities. National Drug Codes (NDCs) must be provided for each product. A firm commitment of availability and price, as well as the source and country of origin, is required just prior to the purchase order. The exact name of the drug, a unique NDC number, and package size must be included in the quote. Expiration dates must be at least 12 months from the delivery date. Substitutions of pack sizes require prior written approval. Partial deliveries require prior written approval. Minimal lot numbers of product are required. A minimum bottle size is 100cc with a child safety cap. Packing slips must include vendor information, VA purchase order number, VA obligation number, NDC, products, and line item numbers. All like products must be packaged and shipped together. Return authorization and pickup for damaged or incorrect products is required within one week of request. Excess quantities may be retained by the government up to $250 in value without compensation. Drug Supply Chain Security Act (DSCSA) compliance is required. The documentation required includes transaction history, transaction information, and transaction statement. All pharmaceutical products must include bar code labeling at the unit-of-use package level. The bar code labeling must be in a linear format that conforms to all **** (formerly EAN. UCC) or Health Industry Business Communication Council (HIBCC) health industry bar code (HIBC) supplier labeling standards. The bar code symbology must comply with all GS1 or HIBCC parameters including, but not limited to symbology type or encoded pattern, bar and space dimensions and tolerances, and allowable ratio of wide to narrow elements. The bar code may be any linear bar code symbology, such as **** (formerly EAN. UCC), GS1 Data Bar (formerly RSS), or Universal Product Code (UPC) if the UPC contains the National Drug Code (NDC). The bar code must encode the NDC, either alone or within the GS1 data structure Global Trade Item Number (GTIN). The bar code printing must be American National Standards Institute (ANSI)/International Organization for Standardization (ISO/IEC) quality grade C or better. Manufacturers and packagers must ensure that production runs include an initial verification check, as well as routine audits, to ensure the bar code is printed clearly and consistently to meet the quality standard of grade C or better. Contractors shall be responsible for ensuring that bar code labels meet the quality requirements specified in this section prior to shipping pharmaceutical products to any authorized ordering activity under this contract. The bar code must be on the outside container or wrapper of the medication as well as on the immediate container, unless the bar code is readily visible and machine-readable through the outside container or wrapper.
