The Navajo Area Indian Health Service, Chinle Service Unit, intends to award multiple blanket purchase agreements (BPAs) for direct patient care supplies. The anticipated period of performance is from the date of award to September 1, 2025, to May 31, 2026. Interested vendors must submit their capability statement, price lists, and catalogs to the point of contact via email no later than September 2, 2025. No further awards will be made unless the need for the Chinle Comprehensive Health Care Facility changes.

The tender is for the rental of heavy equipment. Bidders must provide equipment in satisfactory operating condition, capable of efficiently and safely performing the designated work. An inspection of the equipment will be conducted at the beginning and end of each rental period. If the equipment breaks down, the contractor must repair or replace it within 48 hours. The rental period will be extended by the number of days the equipment is non-operational. The equipment must be returned in the same condition and with the same fuel levels as it was received. Insurance requirements include workers' compensation, public liability, and property damage insurance. The minimum amounts of coverage are $250,000 per person and $1,000,000 per occurrence for bodily injury, including death, and $250,000 per person and $1,000,000 per occurrence for property damage. The contract term is until December 31, 2025. Bids must include a properly executed reciprocal limitations act form, a workers protection and investment certification form, and literature providing the manufacturer, model, and specifications for each piece of equipment. Payment will be made upon satisfactory rental of the equipment for each requested rental period via ACH (direct deposit).
