This is a non-personal services contract for onsite document and hard
drive destruction services for USA MEDDAC at Fort Knox, Kentucky, Blue Grass
Army Depot, and Camp Atterbury, Indiana. The contract includes all personnel, labor, supervision, management support, transportation, equipment, and materials. The contractor must have direct experience in providing onsite document destruction operations for PHI/PII and hard
drive destruction within healthcare facilities in the past 36 months. The contractor must be AAA certified by NAID, bonded, and insured. Liability insurance of $1,000,000 is required. A HIPAA Business Associate Agreement must be signed prior to the contract start date. The government will evaluate contractor performance according to a quality assurance surveillance plan. Specific tasks, standards, and procedures are detailed in the document, including arrival times, certificate of destruction, recycling shredded material, and security procedures. The government will provide some items, and the contractor will provide others, including collection consoles, transport carts, and vehicles. The contract is a firm fixed price (FFP). Contractor personnel must comply with applicable installation, facility, and area commander installation/facility access and local security policies and procedures. Training requirements include antiterrorism, iWatch, controlled unclassified information, and HIPAA training. The contractor must provide a contract manager and alternate with specific experience. Contractor personnel must wear distinctive uniforms and USA MEDDAC identification badges. The estimated annual poundage of PHI/PII shredded is 41,000 pounds, and the estimated hard
drive electronic media to be destroyed annually is 500 pounds. Onsite document destruction services shall be made biweekly twice per month. The award will be to the lowest priced, responsive quote received from a responsible offeror.
