This is a
performance work statement (PWS) for audiology services to be provided to New York Army National Guard (NYARNG) service members. The period of
performance is five years, beginning August 1st. The contractor will provide audiology services at designated locations throughout New York State, at NYARNG armories or military facilities, Monday through Sunday, between **** and **** hours, excluding US holidays and government facility closures. The contractor must maintain an adequate workforce, staff two events simultaneously, and meet specific throughput requirements. Telework is not permitted. The contractor must comply with all installation regulations, including fire, traffic, safety, and security regulations. The contractor must ensure all employees possess required licenses to provide audiology services in New York State. The government will evaluate contractor
performance according to a quality assurance surveillance plan (QASP). The government will provide certain property, materials, equipment, and services (GFP/MES), but the contractor will furnish mobile hearing booths, printing, and other necessary equipment. The contractor must perform auditory testing, maintain medical records, and upload results into the data repository. The contractor will provide monthly invoices for services rendered. The contractor must support all events in New York State, responsible for transportation, setup, and teardown of equipment. The government has unlimited rights to all documents produced under this contract. The contractor must notify the contracting officer immediately if an organizational conflict of interest (OCI) arises. The solicitation will be issued electronically on SAM. gov.
