This is a Request for Quotation (RFQ) for medical services at the Great Onyx Job Corps Center. The services include examinations, assessments, treatment, prescriptions, immunizations, health monitoring, and prevention planning. The contract will be for a base year with four one-year options, and a six-month extension. Offers must be delivered by 17:00 Eastern Time on July 8th, 2026.
Invoices shall be submitted on a monthly basis between the first and fifteenth of the following month and shall be submitted through the invoicing processing platform (IPP).
The government will award a contract to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. The factors are: Technical Capability, Past Performance, and Price.
Key personnel must have an active, unrestricted license to practice independently in the state where services will be provided, maintain licensing requirements, and agree to a criminal background check and drug testing if determined necessary.
Failure on the part of the contractor or its subcontractors to comply with the terms of the clause regarding antidiscrimination and DEI compliance may be grounds for the contracting officer to terminate the contract for default.
Protests must be filed within the timeframes specified in FAR **** agency protests, USDA strives to resolve them within 35 business days of receipt.
Submission of a knowing false statement relating to offerors compliance with requirements may subject the offeror to liability under the false claims act and/or criminal liability.
