Fulton County is seeking a service provider for senior mobile wellness clinic services, including Medicare wellness visits, screenings, and education for seniors aged 65+ with Medicare or public insurance. The provider will offer services such as annual wellness visits, advanced care planning, depression screenings, Medicare preventive services, fall risk assessments, and neurocognitive assessments. Services will be provided at four senior multipurpose facilities. Providers must utilize their own medical billing system, as there will be no direct compensation from Fulton County. Proposals are due July 6, 2026. A pre-proposal conference will be held on June 15, 2026.
Proposals are due by Monday, July 6, 2026, at 11:00 a. m. All proposals, including all attachments, must be received by the county through bidnet direct no later than Monday, July 6, 2026 at 11:00 a. m.
Awards will not necessarily be based on cost alone. The evaluation criteria include Project Plan/Approach to Work (30%), Qualifications of Key Personnel (25%), Relevant Project Experience/Past Performance (25%), Availability of Key Personnel (13%), Local Preference (5%), and Service Disabled Veterans Preference (2%).
Proposers must demonstrate substantial knowledge and experience in providing mobile wellness clinic services. Key personnel must have a minimum of three years of relevant work experience, and the project manager must have a minimum of five years of experience. Financial responsibility and business practices will also be reviewed.
The document mentions Facility Inspection in the table of contents but does not specify if it is mandatory or optional for proposers. It also states the county may request site visits as part of the evaluation process.
Protests must be submitted in writing to the purchasing agent. Protests arising from bases known prior to submission must be within three business days of submission. Protests arising from bases known after submission must be within ten business days after the proposer knew or should have known, but no more than ten business days after award.
Violating the no-contact policy during the procurement process will result in a written finding of non-responsiveness. Submitting more than one proposal as the primary proposer or member of a joint venture for the same work may also lead to disqualification.
