This is a Sources Sought notice for the procurement of a brand name H WAVE H4 multiphasic stimulation device. The Department of Veterans Affairs (VA) VISN 17 intends to establish an Indefinite Delivery/Indefinite Quantity (IDIQ) contract for this device, which is used to support veteran recovery, restore function, and manage pain. The estimated annual demand is **** devices. Deliveries will be on a bimonthly schedule (1st and 15th of each month) to the Sam Rayburn Memorial Veterans Center CPU Bonham VA Medical Center. The contractor will be responsible for maintaining stock and drop-shipping to the CPU. The contract will have a firm fixed price with a base year and one option year. A warranty of one year or more is required. The notice also outlines procedures for obsolescence, replacement items, invoicing (electronic submission via Tungsten Network is required), and records management. The contractor will not access the VA network, and no security accreditation package is required.
The supplier shall deliver all items ordered under the contract during standard operating hours, Monday through Friday, excluding federal holidays, to the following location: Sam Rayburn Memorial Veterans Center CPU Bonham VA Medical Center, Attn: Shellman, VISN 17, **** E. 9th Street Bldg. 10, Bonham, TX **** of the H4 wave devices will be completed on the 1st and 15th of each month.
Itemized billing invoices will be submitted to the VA Financial Services Center (VAFSC) for payment through Tungsten Network no later than 10 working days after each month of service. It is permissible to process payments via the Government Purchase Card (GPC) if the Tungsten billing method is unavailable.
The supplier shall include its express warranty along with the price list, which must protect the government against any manufacturing and any other latent defects for a period of one year or more. During the warranty period, the supplier shall make all necessary adjustments and furnish all labor and materials at no additional cost to the government.
The government intends to issue IDIQs at fair and reasonable prices with qualified sources of supply, i. e. , suppliers.
Total or partial termination for cause of contract, reduction of payment for all remaining invoices, supplier adverse past performance recorded, opportunity will not be provided to supplier for future orders, and potential cancellation of supplier's IDIQ.
