This is a Request for Quotation (RFQ) for blood products and immunohematology reference laboratory services for a 5-year Blanket Purchase Agreement (BPA). The award will be based on the most advantageous quotation considering non-price factors (ability to provide full range of products, response times, inventory, coverage) more heavily than price. The dollar limitation per individual call is $150,000. Delivery is to Lawton Indian Hospital, OK.
The period of performance required is a 5 year bpa ****.
A summary invoice shall be submitted at least monthly, or upon expiration of this bpa, whichever occurs first, for all deliveries made during the billing period.
The government will award a purchase order to the responsible quoter whose conforming quotation is most advantageous to the government, based on a comparative evaluation of quotations against the following factors: quotes will be compared on the offerors demonstrated ability to: provide the complete range of blood products, supplies, and immunohematology testing listed in section 2, 4, 12, 13 of the sow, with the fewest gaps or exceptions. meet or exceed the required response and turnaround times in statement of work sections 4, 12, and 13 maintain sufficient standing order inventory with scheduled change solicitations and credit back for returned units. provide 247 telephone coverage, acceptance of verbal emergency orders, an empowered poc, and consultation services. , and price.
