Key personnel must provide evidence they meet the qualifications stated in the PWS, including resume, degree, evidence of experience, certifications/licenses, DEA license, CPR/BLS/ACLS/PALS certifications, and NPDB report.

The due date for all questions associated with this solicitation is 10 days before the offer due date.

Quotes that fail to provide required administrative documents or reject the terms and conditions of the solicitation may be excluded from consideration. Failure to follow instructions and/or provide required information may render a proposal nonresponsive or unacceptable.

Delivery will be made within a specified number of days after receipt of order, to be completed by the vendor.

Payment is to be made within 30 days after receipt of properly executed invoice or delivery, whichever is later. Discounts for early payment are accepted but will not be an award consideration.

The manufacturers standard warranty will apply unless otherwise specified in the solicitation.

It is the intent of the state to award this contract on an all-or-none basis to the lowest responsive, responsible bidders meeting the specifications.

Bidders must be registered in the LAGov system. Corporations and limited liability companies must be registered and in good standing with the Louisiana Secretary of State for contracts over $25,000.

Failure to deliver within the time specified or to provide services as specified will constitute a default and may cause cancellation of the contract. The state may purchase on the open market and charge the contractor for excess costs.

Bids may be rejected if they do not conform to specifications, if they contain uninitialed corrections, or if they are not submitted on the state's standard forms. Bidders may also be rejected if they have been convicted of or pleaded guilty to certain felonies related to public contracts.
