verification form, specifications, drawings), must be included in the submission. The bid must be manually signed or submitted online via the vendor portal. Bid prices must be typed, written in ink, or submitted online. All bids must be returned in an envelope clearly marked with the bid opening date and number, or submitted online via the vendor portal. The contractor's license number must appear on the bid opening envelope for projects over $50,000 or involving hazardous materials. Any objections to the classifications must be sent to the Louisiana State Licensing Board for Contractors and the Office of State Procurement no later than 10 working days prior to the bid opening date. Bids must conform to all applicable federal and state laws and regulations and specifications. Bidders must specify the brand and model number of the product offered. Bidders proposing an equivalent brand or model should submit descriptive information. Bid prices must be firm for a minimum of 30 days. Vendors are responsible for including all applicable taxes in the bid price. All products must be new, never previously used, and the current model and packaging. The State of Louisiana may terminate the contract for various reasons, including misrepresentation, fraud, or breach of contract. Failure to deliver within the specified time or to provide services as specified will constitute a default. All contracts shall be construed in accordance with Louisiana law. Bidders must comply with civil rights laws. Any qualified individual with a disability who desires to attend the bid opening must notify the office in writing no later than 7 days prior to the opening date. The contractor agrees to indemnify the state from claims arising from its fault. Evidence of authority to sign and submit bids must be provided. The state may reject a proposal from a business in which any individual with a 5% or more ownership interest has been convicted of a felony related to the solicitation or execution of a contract. The contractor agrees to adhere to the Copeland Anti-kickback Act, Clean Air Act, Clean Water Act, Energy Policy and Conservation Act, anti-lobbying and debarment act, and prohibition of discriminatory boycotts of Israel. The contractor shall comply with federal and state disabilities laws and regulations and accessibility requirements. All bid amounts shall be submitted in US dollars. The bidder certifies that the price submitted was independently arrived at without collusion.
