The dental hygienist contractor shall possess graduation from an accredited dental hygiene program, current unrestricted dental hygienist license, current certifications required by state law for anesthesia or nitrous oxide administration if applicable, minimum of one year clinical experience preferred, and current BLS certification.
Any false or misleading information submitted by an enterprise when submitting an offer in consideration of an award set-aside under the authority of the Buy Indian Act is a violation of the law and punishable under 18 **** claims submitted as part of contract performance are subject to the penalties enumerated in 31 **** to **** and 18 U. S. C. 287.
Failure to maintain qualifications, licensure, credentialing, or privileges may result in immediate termination of the contract without cost to the government.
