Failure to comply with terms and conditions or unsatisfactory performance can lead to suspension. Minor violations may result in suspension for 6 months after three vendor performance reports within 12 months. Major violations can lead to immediate suspension for 6 months. for major violations, as deemed by mphas contracting officer, which may include but is not limited to failing to maintain proper insurance, subcontracting work without mphas authorization, failing to pay prevailing wages, or failing to correct unsatisfactorily completed work within the requested time, the contractor will be immediately suspended for a 6month period.

Failure to abide by communication requirements, submitting proposals electronically after the deadline, or failing to demonstrate compliance with required contracting standards may lead to disqualification or rejection. failure to abide by this requirement may be cause for mpha to not consider a proposal submittal.
