Personnel security investigations are required for all contractors entering the site. Upon 'notice of contract award', all contractors, subcontractors, their supervisors in charge, and all working personnel requiring access to the site shall be required to submit to a background security investigation in accordance with the Department of Commerce security regulations.

Bids must be received by 3:00 PM on Monday, June 15, 2026. Any bid received by the SCCOE after the bid deadline shall be returned to the bidder unopened.

Periodic payments will be made within thirty (30) days of SCCEO's receipt of an application for payment. Payments to contractor for work performed will be made on a monthly billing basis.

Contractor warrants all work for a period of one year or the manufacturer's warranty period, whichever is later, after final acceptance. Contractor warrants all work under the contract. . . to be of good quality and free from any defective or faulty material and workmanship.

Liquidated damages of **** per calendar day of delay beyond the completion date apply. Contractor shall pay to the SCCEO as fixed and liquidated damages. . . the sum of five hundred dollars (****) for each and every calendar day of delay beyond the time prescribed. . .

Attendance at the pre-bid conference and all site visits is mandatory. Attendance is mandatory for the entire prebid conference and all sites.

Failure to attend the mandatory pre-bid conference or site visits will result in disqualification. Any bidder who fails to attend the prebid conference or any portion of the site visits, or who does not sign in at each location, will be disqualified from work on the project.

The contractor shall ensure that all employees and/or subcontractors have adequate knowledge of commercial cleaning chemicals, equipment, and techniques necessary to perform work.

In the event of a noshow by the cleaning contractor, the normal cleaning daily items may be deducted from any amounts due and owing under this contract.

Work can only be completed during business hours Monday-Friday, 7:30 a. m. - 4:00 p. m. , excluding major holidays. Sample photos of completed work to be submitted with invoice and onsite approval from Resident Services Coordinator and Supervisor no later than 10 a. m. on July 2, 2026.

Contractor shall submit invoices for payment in a format acceptable to MDHA and shall submit invoices no more frequently than monthly for satisfactory and accurately performed services. Contractor shall be paid as work is completed and invoices are approved by MDHA. All payments shall be effectuated by automated clearing house (ACH). Invoices shall be submitted to accounts payable@nashvillemdha. org.

Offeror warrants that all goods under this quote shall be new, unused, current production models, where applicable and shall be free of defects in materials, design, and manufacture for the duration of the resulting purchase agreement. Contractor warrants that for a period of one year from date of delivery and/or installation, whichever is later, the goods provided shall be free of any defects.

Quotes will be evaluated and ranked on the criteria below that best fits MDHA's needs in MDHA's sole discretion. Ordinarily, quotes will be evaluated to identify the lowest responsible offeror.

MDHA is seeking proposals from qualified proposers. If awarded, MDHA will request any required licenses and insurance certificates.

If contractor fails to satisfactorily provide cure within thirty (30) days of breach identification, MDHA shall have the right to immediately terminate this contract and withhold payments in excess of fair compensation for work completed. Such termination shall not relieve contractor of any liability to MDHA for damages sustained by virtue of any breach by contractor.
