Additional required information includes name of business, address, areas of practice, description of physicians education, licensing, experience, qualifications, number of years with the practice, references, examples of record of success as a police physician, ability to provide services in a timely fashion, cost details, statement of ownership disclosure, disclosure of investment activities in Iran, certification of noninvolvement in prohibited activities in Russia or Belarus, business registration certificate, and a statement certifying no prior or pending ethics complaints.
