Respondents should address their demonstrated experience with specific federal contract examples, including contract number, agency, period of performance, approximate dollar value, and a brief description of the work performed. Key personnel roles require specific certifications.

Late proposals will be automatically disqualified. Communication with OPERS staff outside of the specified channels may result in disqualification.

Liquidated damages may be assessed if the contractor fails to cure delays or nonperformance within the specified time. When the contractor is given notice of delay or nonperformance as specified in paragraph 11 termination for default and fails to cure in the time specified, it is agreed the contractor shall pay to hhsc the amount, if any, set forth in this agreement per calendar day
