Applicants must have reviewed the attached independent contractor agreement and must be able to meet the counties insurance and w9 requirements, as stated in the bid notice.

If contractor fails to comply with any material provision of the agreement, contractor shall be liable for all damages, including with limitation, the cost of procuring similar supplies or services and all other costs and expenses incurred by the county because of such failure, as stated in the bid notice.
