The vendor is responsible for including all applicable taxes in the bid price. All products must be new, never previously used, and the current model and/or packaging. The manufacturers standard warranty will apply. The state of Louisiana has the right to terminate the contract for any of the following reasons: misrepresentation, fraud, conflict of contract provisions, abusive conduct, intentional violation of the procurement code, or any listed reason for debarment. The state of Louisiana may terminate the contract for convenience at any time by giving 30 days written notice. Failure to deliver within the time specified or to provide the services as specified in the bid will constitute a default. All contracts shall be construed in accordance with and governed by the laws of the state of Louisiana. By submitting and signing this bid, the bidder agrees to abide by the requirements of various civil rights laws. The contractor warrants it will comply with federal and state disabilities laws and regulations. The contractor agrees to promptly respond to and resolve any complaint regarding accessibility of its products and services. The contractor agrees to indemnify and hold harmless the state from any claims or damages arising out of its failure to comply with the requirements of this paragraph. Evidence of authority to sign and submit bids must be provided. Any public entity is authorized to reject a proposal or bid from, or not award the contract to, a business in which any individual with an ownership interest of five percent or more, has been convicted of, or has entered a plea of guilty or nolo contendere to any state felony or equivalent federal felony crime. The State of Illinois, Department of Corrections, is seeking bids for an as-needed contract for the rental of an electric forklift truck at the Big Muddy River Correctional Center. The contract is for three years, commencing July 1, 2025, and ending June 30, 2028. Bids must be submitted no later than June 20, 2025, at 11:00 a. m. via Bid Buy or email. The vendor must remain firm for 120 days from opening. The contract includes delivery, setup, training, and removal of the forklift. Detailed specifications are included in the attached contract. The vendor is responsible for all transportation, handling, and maintenance of the forklift. Security background checks for all on-site employees are required. The State is not obligated to award a contract. If awarded, the contract will be awarded to the responsive and responsible vendor who submits the lowest price.

The Federal Bureau of Prisons intends to award an indefinite delivery/requirements type contract for solid waste removal services at the Federal Correctional Institution (FCI) Pekin and the Federal Prison Camp (FPC). The contract will have a base year and four 12-month option years, plus a 6-month extension. The contractor is responsible for loading, unloading, hauling, and disposing of refuse in accordance with all local, county, state, and federal regulations. The government reserves the right to award without discussion. The contractor must provide a 24/7 means of contact, and the government has the right to inspect disposal sites. The contractor must use a Class I disposal facility. The contractor must provide two sealed self-contained compactors. Monthly invoices must include weight tickets. Contractor personnel must present photo identification. The contractor must provide written notification within three business days of any permit issues, major modifications, or management changes. The estimated annual waste is approximately 850 tons. The contractor must pick up one 38 cubic yard compactor from the FPC on the second and fourth Wednesdays of each month, and one 38 cubic yard compactor from the FCI on Mondays and Thursdays each week. The number of pick-ups and total weights are estimates. The waste material will consist of institutional waste, solid and/or wet waste from the food service department and other departments within the institution. All contractors doing business with the federal government must be registered in the System for Award Management (SAM) database.
