Bidders must submit safety records, including citations for serious, willful, or repeat violations from MIOSHA within the prior 36 months. They must also disclose any violations of state, federal, or local laws, prevailing wage laws, or workers' compensation laws within the past 5 years, and any disbarment. To demonstrate its qualifications for the project, the bidder must submit safety records. Safety records shall include citations for all serious violations, willful violations, and/or repeat violations issued by the Michigan department of licensing and regulatory affairs, Michigan occupational safety and health administration MIOSHA within the prior 36 months.
Failure to meet the final completion date will result in liquidated damages of $800 per calendar day. Failure to meet such date will result in the assessment of liquidated damages in accordance with section **** of the MDOT 2020 standard specifications for construction.
