The solicitation is for the provision of prosthetic and orthotic services at the Federal Medical Center in Lexington, Kentucky. The contract is an indefinite delivery/requirements type contract with firm fixed unit prices. The contract includes a base year period from the effective date of award (October 1, 2025) through 12 months from the effective date of award, with four possible 12-month option year renewals. The contractor must be located within a 75-mile radius of the FMC address. The contract individual must have at least three (3) years of experience in this field. The contractor shall ensure submitted prospective employees meet the requirements of vouching process as described in the human resource management personnel manual 3000. 03. The individuals name, work experience, and proof of qualification must be provided at the time the quote is submitted. Failure to submit this information may result in the quoter not being considered for award. The contractor must maintain medical liability insurance in the amount of $1,000,000 per occurrence/$3,000,000 aggregate. The contractor must complete and submit an appropriate separation checklist to the contracting officer before any employee or subcontractor employee terminates working on the contract. The contractor must ensure that all employees take appropriate privacy training, including subcontractors who have access to PII as well as the creation, use, dissemination and/or destruction of PII at the outset of the employees work on the contract and every year thereafter.
