The contractor must be located within 25 miles of the Austin State Supported Living Center. The HHSC reserves the right to change, consolidate, delete or add service locations. The contractor must provide 24-hour contact information for at least two personnel. All personnel must comply with applicable state rules, regulations, and HHSC requests regarding personal and professional conduct. The contractor must maintain required insurance coverage, including workers compensation, commercial general liability, commercial automobile liability, professional liability, and umbrella liability insurance. The contractor must submit pricing sheets, proof of board certification, proof of malpractice and liability insurance, and a completed workers protection and investment certification.
