The Indian Health Service, Kayenta Service Unit, Kayenta Health Center, is seeking a blanket purchase agreement (BPA) for various endodontic supplies for root canal therapy. The Texas Health and Human Services Commission (HHSC) is seeking bids for endodontic services at the Austin State Supported Living Center. Bidders must submit a comprehensive response including all required information and documentation by the response deadline (****). Questions and clarifications must be submitted by 20250825. The contract will begin upon award and expire on August 31, 2026. HHSC may renew the contract for up to two one-year periods. Additional information regarding the requirements and submission instructions can be found in Appendix A.

Responses to this IFB must be received by August 13, 2025, 10:30 AM Central Time. Questions or clarifications submission deadline is August 5, 2025, 5:00 PM Central Time. Responses to questions or clarifications addendum posted August 8, 2025, 5:00 PM Central Time. The anticipated contract start date is September 1, 2025. Bidders must submit a comprehensive response including all required information and documentation. The scope of work includes diagnosis, prevention, and treatment of diseases and injuries related to dental pulp, endodontic retreatment, cracked teeth, root canal therapy, tumor removal, and extractions. Services must be performed at the contractor's location. HHSC reserves the right to change, consolidate, delete or add service locations with 30 days advance written notice. Contractor shall perform contracted services, including any minor adjustment callback services, Monday through Friday between the hours of 8 a. m. and 5 p. m. Central Time. Contractor shall have an active telephone service answered during normal business hours. Contractor must provide the HHSC contract manager with 24-hour contact information for a minimum of two contractor personnel. All personnel must comply with all applicable state rules, regulations, and HHSC requests regarding personal and professional conduct. The use or possession of any kind of intoxicants or illegal drugs by contractor or any of its subcontractors employees while on duty at HHSC premises is prohibited. All HHSC facilities are nonsmoking buildings. Contractors and subcontractor employees are prohibited from smoking in all nondesignated areas. Contractors must wear company uniforms and identifying badges. Bidders must submit documentation of experience and qualifications. Bidders must provide a minimum of three verifiable references for current or previous contracts of the same size and for similar or same services within the last three years. The awarded contract will be effective on the date the contract is issued and will expire based on the schedule for delivery of services. The initial contract term is from September 1, 2025, to August 31, 2026. HHSC, at its sole discretion, may renew the contract for up to two one-year renewals. The HHSC may extend the term of the purchase order or other contract beyond the initial term and all exercised renewal periods for up to one year as necessary. All communications related to the contract and requests for changes to the contract will be processed through the designated HHSC contract manager. The HHSC agency will monitor all work performed by contractor and shall regularly communicate with the contractor to address questions, concerns or progress. Contractor will be notified by the HHSC contract manager or designee in writing to correct any service or portion of a service. Contractor must submit invoices for services performed on a monthly basis. Contractor may be required to submit bond documentation and current certificates of insurance or other proof acceptable to the HHSC at the time of notification of a potential award. Failure to timely submit acceptable proof may result in the HHSC's revocation of the award. Contractor shall maintain the required insurance during the initial contract term and any renewal or extension period exercised. Contractor shall be responsible for ensuring its subcontractors are in compliance with all applicable insurance and bond requirements.
