The Department of Human Services is seeking a vendor for FY26 barber and beauty services at the Elisabeth Ludeman Center. A mandatory bidder conference is scheduled for June 25, 2025, at 2:30 PM at the Ludeman Center. The contractor must be licensed by the state of Illinois as a barber or cosmetologist. Services will be provided on an appointment basis, a minimum of nine hours per week, between the hours of 6:00 PM and 9:00 PM weekdays and between 10:00 AM and 5:00 PM on Saturdays. The contractor shall submit signed contractual services time register forms on the 15th and last day of each month for payment of services. The contract is anticipated to last until 6/30/2026. The vendor shall not perform services, provide supplies or incur expenses which exceed the contract amount unless a higher amount is authorized in writing by the state prior to the vendor performing the services, providing the supplies, or incurring the expenses. The contractor shall observe all traffic regulations of the center and lock unattended vehicles. Cars in violation of facility regulations may be towed from grounds as owners expenses. The contractor shall observe and obey all rules and regulations governing the behavior of the centers employees, including state, civil, and local center rules and regulations.

This is a sole source contract for the purchase of customized forensic reagents and consumables for the identification of fallen US service members. The requiring activity is the Defense Health Agency (DHA), Armed Forces Medical Examiner System (AFMES). Promega Corporation is the sole source. Interested parties must submit technical data, including cost, to determine capability in providing the same product by 10:00 AM EST on June 12, 2025. Late responses will not be reviewed.

Proposals must be submitted via the Ohiobuys website by 2:00 PM EST on the date indicated on the sourcing project. The RFP consists of 44 pages. All inquiries must be submitted through the Ohiobuys website. The proposal must include a technical proposal and a cost proposal, outlining how each deliverable will be completed within the specified timeframes. The technical proposal must include a transmittal letter, organizational experience, technical approach and work plan, personnel qualifications, and samples of previous related projects. The cost proposal must include costs for each service (hair cut, shampoo/conditioner, shampoo/conditioner/cut, beard trim), and travel costs are included in the deliverables. A standard affirmation and disclosure form and contractor information form must be included with the proposal. The evaluation process may consist of up to four phases: initial review, evaluation committee review, request for more information, and negotiations. Evaluation criteria include cost proposal, bid packet completion, and history of successfully managing other contracts. The minimum acceptable score to award a contract is 325. Any contracts resulting from this RFP are subject to the terms and conditions of the personal services contract. Payments are contingent upon the availability of state and federal funds. A copy of the contractor's license is required. The contractor must not be subject to an unresolved finding for recovery under section 9. 24 of Ohio Revised Code. No contract funds may be spent offshore.
